We work with primary mental health providers across the different levels of care from acute service providers to outpatient service providers. We are a great option for individuals stepping down from in-patient care or residential programs as well as individuals needing intervention to avoid higher levels of care.
Though we do not work with individuals with a primary substance abuse disorder, we can step in to provide support to the family system as well as offer treatment for co-occurring mental health diagnoses.
Adolescents in crisis that need to be able to access care that works with their school schedules and their parent's work schedules.
College students wanting robust support on campus rather than taking a medical leave while completing their studies.
Clients with chronic and persistent mental health challenges who need the support of a team familiar with navigating chronic suicidality, psychosis, or other serious mental health issues that get in the way of daily activities and self-care.
Clients discharging from inpatient hospitalization, residential programs, aftercare programs, as well as traditional therapeutic settings like PHPs and IOPs who need support re-integrating into key life roles (family, school, work, hobbies, spirituality).
Clients utilizing traditional outpatient services like individual therapy, family therapy and medication management but are experiencing more significant distress and would like to avoid hospitalization or the need to move away from their life and loved ones.
We are a primary mental health program designed to support adolescents and adults with emotional and psychological pain. The Mind-Body Collaborative uses an evidence-based Program for Assertive Community Treatment (PACT) approach to create paths forward.
We offer community-based services to ensure that our clients are able to stay within their environment and continue their daily lives without being disrupted by on-going treatment. Additionally, to further remove barriers, we will do in-home therapy sessions when necessary or appropriate. Services are provided by a mobile, interdisciplinary team of psychiatrists, therapists, dieticians, recovery coaches, and a vocational coach.
We are an out-of-network provider. However, we do supply superbills for clients to submit for reimbursement to their insurance. Usually, we are seeing individuals and their families for a minimum of 20 hours per month.
If you would like to make a referral or learn more about our program, please call 737-256-7929 or email Info@thembcollaborative.com.
